Survival and Patency of Saphenous Vein Grafts in a Xenograft Model

چکیده
The saphenous vein graft (SVG) has been the bypass conduit of choice for over 50 years, but paucity usable autologous remains as limiting factor. We hypothesize transplanted allografts can be used patients with limb-threatening ischemia. A major factor to patency a SVG is immunologic remodeling process, promoting formation intimal hyperplasia; primary cause stenosis and failure. Our goal evaluate new innovative strategies protect from host immune system using rat xenograft model. This accomplished am institutional review board/Institutional Animal Care Use Committee-approved model in which human segments are grafted into rats infrarenal interpositional aortic grafts, after subjecting veins various treatments intended extend patency. reverse accelerates rate failure due extreme difference species biomarkers. have performed SVGs on 47 Sprague-Dawley rats, either no pretreatment, or pretreatment fixatives (glutaraldehyde), decellularizing agents (sodium dodecyl sulfate) at varying concentrations periods time 2 weeks 9 months. Immunofluorescence immunohistochemistry being determine precise localization molecular determinants how they evolve occurs time. Preliminary data our laboratory shows strong reaction implantation SVG, expected. Control (SVGs pretreatment), show degradation early grafting. All pretreated vessels showed greater luminal improved histological appearance compared controls, 3, 6, months; some impeded any progression hyperplasia even also seeing novel cells infiltrating recreating cell wall matrix decellularized graft. successfully tested hypothesis that subjected such when implanted will retain physiological performance controls. research significant implications potentially improving retention future current clinical applications. potential expand availability other humans, animals.

Understanding saphenous vein graft patency.
Saphenous vein was the conduit used in the first series of coronary surgery, and, with the exception of revascularization of the left anterior descending coronary artery, it remains the most commonly used conduit.1 There are several reasons for this. First, because of its relatively large diameter and wall characteristics, it is technically easy to use; second, it is plentiful, and therefore ca...

متن کاملImpacts of Intraoperative Flow on Graft Patency of Sequential and Individual Saphenous Vein Grafts
OBJECTIVE We sought to delineate the predictor of saphenous vein graft (SVG) failure and to evaluate the impact of sequential grafting of SVG on graft flow as the significant predictor of patency. METHODS Angiograms and clinical records of 439 patients who underwent coronary artery bypass grafting with aortocoronary SVG were reviewed. Of these, 708 distal anastomoses were created by 480 SVGs....
متن کاملA noninvasive technic for determining patency of saphenous vein coronary bypass grafts.
A noninvasive method for determining patency of saphenous vein coronary bypass grafts has been developed in our laboratory. The system consists of a pulsed ultrasonic Doppler, a position-sensing arm for the Doppler transducer, a memory oscilloscope, an electrocardiogram, and strip-chart recorder. Patients are studied with the transducer in the suprasternal notch and intercostal spaces. Wherever...
متن کاملOne-year patency control and risk analysis of eSVS®-mesh-supported coronary saphenous vein grafts
BACKGROUND The eSVS® external venous nitinol mesh (Kips Bay Medical, Minneapolis, USA) was designed to improve long-term patency of coronary saphenous vein grafts (SVG) by preventing pressure-induced wall stress and reactive neo-intimal hyperplasia. We present one-year-patency rates of meshed SVGs assessed by coronary computed tomographic angiography (cCTA). PATIENTS AND METHODS Data from con...
